That is a prolapsed anus/rectum. This can happen by purposely "baring down" in people that have had tooons of anal penetration and stretching. Also, the sucking could be doing this.

This is not a medical emergency or anything. Once the jar is removed the rectum will return into the body. Sometimes with excessive force this will remain like this and surgery will be required to correct it.

The anus and rectum are lined with mucous membrane so perhaps the liquid is just the mucous? I am not sure.

This is very dangerous, because he could easily rupture something and bleed out and never even know it because the blood would go into his abdominopelvic cavity rather than out his asshole. I guess he enjoys this, but really I cannot see how. That liquid is very discusting.
