I was thinking at first this looks like a hermaphrodite. I don't think it could be testosterone affected though, because girls don't pee out of their clits (that structure is purely for sensation, oh and to direct the pee i believe, but i think that's mainly because the urethra is located so close to it ), and well that thing in the picture appears to have the head of a penis and an opening which would be to the urethra, so yeh, clits don't magically transform into penises, they might elongate or get bigger due to testosterone though, don't know enough about it though
